I have a 3.5mm jack wired to a telephone line. I can hear the music, but only barely, and there is a very loud humming noise in the background. I have tried an isolation transformer and an audio output transformer but have only heard minor improvements. Perhaps I’ve wired them wrong?

You’re on the right track.
A transformer is the ticket. But more specifically, a 600ohm phone line transformer.
The phone side is 600ohm, and if I’m thinking right, the other end is at least 10k.
Believe it or not, before the internet, the phone line was the only game in town to transfer music long distance (analog as it was).
